This article explains how to publish a Qualtrics survey in a Canvas course as an assignment.

Add an assignment to a Canvas course
First, go to the Assignments tab in the Canvas course that you would like to add the survey to. Then click the “+ Assignment” button in the top right corner to add a new assignment. Next, give your assignment a name. We recommend leaving the space below it for an assignment description blank, as it can mess up the formatting of the survey. If you would like to add a survey description or instructions, add these instead as a text question in the Qualtrics survey itself.
In the Points section, we recommend setting the available points to at least 1. If the points are set to 0, the survey may not display properly in the grade book. You can then select if the grade is displayed as points, complete/incomplete, percentage, or other options. If you would like the score to not count towards the final grade, click on the checkbox shown called "Do not count this assignment towards the final grade."
Add a Qualtrics survey to the Canvas assignment
In the Submission Type section of the Canvas assignment, set it to External Tool. Next, click the “Find” button and search for the Qualtrics LTI.
See the GIF below for the steps to configure the external tool. These steps are necessary for adding your Qualtrics survey to the Canvas assignment. Once you have searched for and added your survey, and set (optional) announcements and reminders, be sure to click “Save.” Lastly, click “Select” when you return to the external tool pop-up window to finish connecting your survey to the assignment.
Note that because the submission type is an external tool, it is not possible to use the Qualtrics LTI to make a survey part of a Canvas group assignment.
Finally, finish configuring your assignment settings by selecting submission attempts, anonymous grading options, and who the survey will be assigned to. Once you have completed all the assignment settings, you can either “Save” the assignment or “Save & Publish” the assignment with the buttons in the bottom right-hand corner.
Once the assignment is saved, you can check the details of the added survey. These details are only visible to you; if a learner opens the assignment they will see the Qualtrics survey instead.Congratulations! You have now added a Qualtrics survey to a Canvas assignment. The steps detailed above have shown you how to add surveys to individual assignments.
